An AI form builder for surveys, quizzes and sign-ups with a public link

Describe the form and Quad builds it, with conditional questions, quiz scoring and a public link. Read responses as a summary, then export to CSV or a sheet.

From a sentence to a form people can fill in

"An RSVP for our team offsite on the 14th. Ask if they are coming, and if so how many guests and any dietary needs." That is enough. Quad builds the form beside the chat with a yes or no question, a number field and a multiple choice list, and shows the follow-up questions only to people who said yes. You can reword a question by hand, drag it into a new order or ask the AI to "make it shorter" or "add a question about travel". When it reads right, publish it and copy the link. Anyone with the link can respond without an account.

Question types and settings

Question types (What you get: Short and long text, email, phone, number, date, single choice, multiple choice, dropdown, rating, scale, yes or no, and section headers). Logic (What you get: Show a question only when an earlier answer matches). Pages (What you get: Split a long form into pages with section breaks). Quizzes (What you get: Mark correct answers, give points and show each person their score after they submit). Responses (What you get: Collect email addresses, limit to one response per person, close the form with a custom message). Finish (What you get: Custom submit button text, confirmation message and accent color). Size (What you get: Up to 100 questions, 50 options per question and 10,000 stored responses per form).

Reading and exporting responses

A summary view shows how answers split for each question, with averages for ratings and scales. An individual view lets you read one response at a time and delete any you do not want. Export every response to CSV, or open them in a new Quad sheet to sort, chart and analyze with the AI. Ask the AI in the chat to summarize open-text answers or pull out the most common complaints. Responses are visible only to you, the form's owner. The form itself is public once you publish it, which is the point. You can stop accepting responses or turn the link off at any time.

Templates to start from

Event RSVP with guests and dietary needs shown only to people who are coming. Customer feedback with a satisfaction rating, an NPS scale and open feedback. Workshop sign-up with names, emails and session choice. Contact form with a topic dropdown and a message. A scored quiz with instant results. A two-page job application covering the applicant and the role. Or skip templates and describe the form. A template is just a starting point the AI can rewrite.

When to use a dedicated form tool instead

You need file uploads, payments or signatures in the form. Quad forms do not collect them. You need answers to flow automatically into a CRM or another app. Quad exports CSV and sheets rather than syncing. You collect sensitive data under a compliance regime that requires a specific vendor. Quad forms never ask for passwords, card numbers, bank details or government ID numbers, and the AI will not add those questions. You expect more than the stored response limit on a single form. For RSVPs, internal surveys, class quizzes, sign-ups and quick feedback, building the form in the same chat where you planned the event or wrote the survey is usually faster. Forms are part of signed-in accounts on every plan, Free included.

Frequently asked questions

Can AI make a form for me?

Yes. Describe the purpose and audience and Quad drafts the questions, chooses field types and adds conditional logic. You can edit by hand or ask for changes, then publish it on a public link.

Do people need an account to fill in the form?

No. Anyone with the link can open the form and respond. You can ask for an email address and limit each person to one response in the settings.

Can I make a quiz with automatic scoring?

Yes. Turn on quiz mode, mark the correct answers and set points. Each person sees their score right after they submit, and the score is included in the export.

How do I export form responses?

Export them to CSV, or open them in a new Quad sheet where you can sort, chart and ask the AI to analyze them. The sheet then exports to Excel.

Is the AI form builder free?

Forms are included on every plan, Free included, with a free account and no credit card. Building one with the AI uses your monthly credits like any other request.
